IMPORTANT: Both Membership and SEC NAV Forms must be printed and RECEIVED BY December 20, 2024.
ATTENTION: To prevent the most common errors on the SECNAV form, please make sure:

#13 & #14: In the Identification section, you will need to check both the Social Security No. and State ID/Driver’s License boxes, and provide all the appropriate information.

#29: Height should be in inches. i.e. 5’4” should be 64”.

#24: Base Sponsor’s Name is: Peter Cruz, Chief of Protocol, 15th Wing, PAF. Sponsor Phone is: (808) 368-7854

#27: Leave all boxes unchecked.

#28: Check “No,” then make sure to initial. You can either type in your initials OR hand write it on the form after you print it out.

After your form is complete, print it out and #30: Hand write the Date and Signature to complete the form.
